How do I schedule an appointment?
Appointments are made by ringing (02) 9893 9494 and speaking to the secretary. 


Do you require payment in advance?
Yes. Tax invoices are issued usually one month in advance. The fee is required to be paid in full prior to the appointment date. The performance of the assessment and its recording and production completes the transaction.


What is your cancellation policy?
Late cancellation or non-attendance fees do apply (50%). Please discuss with the secretary before booking appointment.


Where do I send medical records?
Documents must be sent to:  PO Box 3034, North Parramatta NSW 1750
                                                  or   DX 8202, Parramatta 


Where do you conduct your appointments?
Only at 19 Grose Street (Cnr Grose & Villiers Streets), North Parramatta. 


When are medical records required?
All paperwork: briefing letter together with enclosures of treating doctors' reports, reports of x-ray, CT, MRI or other imaging studies must be received no less than 5 days prior to the appointment date.


Do you have a pre-examination history form?
Yes. We ask that the client be at the consulting rooms no later than 10 minutes before consultation time to complete this form.


How long will the evaluation take?
We allow an hour.


Are x-rays needed?
Appropriate imaging studies are needed to validly assess impairment.  We ask that all relevant films be brought on the day of appointment.  New x-rays are sometimes required (x-ray services are conveniently loacted close by) 


How long does it take to return your reports?
Usually 10-14 working days
